A recognized challenge for Human Reliability Analysis (HRA) and Probabilistic Safety Assessment is the systematic, comprehensive and efficient treatment of Errors Of Commission (EOCs), i.e. actions inappropriately performed that aggravate the plant condition and/or the accidental scenario (USNRC 2000, Forester et al. 2007). Quantification of EOC probabilities is especially problematic for inappropriate decisions. Data is rarely available, because of the strong influence of the context (Reer et al. 2004). As a result, some methods quantify these errors via direct elicitation of probability from experts, e.g. Forester et al. (2007) and Le Bot (2004).
